All registration changes (leave of absence, withdrawal, add, drop, change in grade mode, etc.) must be submitting in writing, using the appropriate form listed below.
Note: Deadlines will be adjusted when adding/dropping courses that begin later in the semester.
1 Complete the Returning Student Request PDF and obtain your Program Director's signature.
2 Complete the Leave of Absence Request Form PDF and obtain your Program Director’s signature.
3 Complete the Withdrawal Form PDF and obtain your Program Director’s signature.
4 Complete the Change in Registration Request (Add/Drop/Change) PDF form and obtain a signature from your Program Coordinator, Advisor, or Program Director. * If dropping ALL courses in the semester, complete the Withdrawal Form PDF and obtain the required signature.

WP – Withdraw Pass
Students who will complete certificate, MS or PhD requirements must visit the Information for Graduating Students page for required forms and deadline information
